Loss and Damage Claim Process

 

Crawford & Company (Canada) Inc. is the claims administrator on contract with Global Affairs Canada (formerly the Department of Foreign Affairs, Trade and Development Canada - DFATD) to administer all the loss and damage claims submitted under FSD 15 – Relocation.

 

This document provides the guidelines for filing the Notice of Intent to Claim form and Claim form.

 

Claims Process:

 

1) Notice of Intent to Claim Form:

If loss or damage occurs, employees must submit a Notice of Intent to Claim form to Crawford & Company (Canada) Inc. within 30 days of the receipt of the effects. If effects are received on separate dates, the 30 day time limit applies to each shipment separately. Claims received past the deadline will not be accepted.

 

You may download or print the Notice of Intent to Claim Form from the Administrator’s website at cfscs.ca or email the Administrator at ccas@crawco.ca to obtain a copy.

 

As per FSD 15.17.4, a claim for loss and/or damage under FSD 15 can only be made if the employee submitted a detailed inventory of their household effects to be shipped and/or stored to their FSD Client Advisor prior to relocating. As such, the claim will only be allowed to proceed once the receipt of the inventory is confirmed to Crawford by Global Affairs Canada.  Further instructions on the claims process will be provided once the receipt of the inventory is confirmed.

 

Note: If an employee’s claim is under $200, the Notice of Intent to Claim Form is not required but a claim form should be filed with Crawford within 30 days of receipt of the shipment.

 

2) Claim Form:

The completed claim form must be received by Crawford within 60 days of the date of submitting the Notice of Intent to Claim form. Claims received past the deadline will not be accepted. If a Notice of Intent to Claim form was not submitted, the claim will not be accepted.

 

Employees must ensure that all damaged and missing items are included on the claim form submitted as this constitutes the full and final claim. Additions to the claim will not be accepted.

 

Documents to forward with the completed claim form are: pages of the pre-move valued inventory on which damaged or missing items are listed, copy of the moving company’s delivery documents detailing any missing or damaged items and any pictures available showing the damage sustained.

 

Note that a separate claim must be submitted for each portion of the relocation. For example, if an employee has a surface, air and a long-term storage shipment, a claim for each shipment must be submitted.

 

Crawford will review the completed claim form within 5 days of receipt and will contact the employee if any information is missing. If necessary, Crawford will contact the employee to arrange an appointment with a claim appraiser.

Note the following:

  • A copy of the Notice of Intent to Claim Form/Claim Form will be submitted to the carrier by Crawford to notify them of the claim being made.
  • Damaged items must be kept until release has been authorized by Crawford.

 

Loss or damage claims for Personal Motor Vehicles (PMVs)

 

For PMVs shipped to and from Canada, for PMVs shipped during a cross posting, or for PMVs in long-term storage: Follow the procedures outlined above.

 

For vehicles shipped from a third location:

If a vehicle is shipped from a third location, employees are responsible for ensuring that all-risk ocean cargo insurance coverage is placed on the vehicle. Should loss or damage occur, employees must submit a claim to the insurance company which provided the all-risk ocean cargo insurance.
